Dr. Todd Bovenizer-Complete Family Orthodontic Care

FIRST VISIT

Patient Forms

We look forward to meeting you on your first visit to our office. During this time, we will help you get acquainted with our office and learn about your orthodontic treatment. We understand that each patient's orthodontic treatment is different, and we promise to give you the personal attention you deserve. Your first orthodontic visit will consist of an examination and discussion of potential treatment options. This important visit will address your orthodontic needs to determine if this is the proper time to begin treatment.

After we have evaluated your records, we conduct a treatment consultation to explain all aspects of your treatment in detail. We will provide a complete clinical diagnosis regarding the nature of the problem, a written summary of the findings and a detailed plan that outlines your treatment, goals and estimated fees. We encourage questions from both the patient and the parent, and as such, we provide ample time to answer your questions.

Your complimentary exam with Dr. Bovenizer includes a digital X-ray and facial photographs. While in our waiting area, patients can enjoy complimentary coffee, tea or water while enjoying wireless Internet access. Kids can test their video game skills in our game room. Dr. Bo has XBOX 360 and is anxiously awaiting PlayStation 3. He may even challenge you to a game! We have an exciting “Bovie Bucks” rewards program for patients who practice good oral hygiene, wear their rubber bands, make good grades in school and tell their friends about Dr. Bo. These “Bucks” can be redeemed for prizes like I Pods, I-Tunes gift cards, ACC merchandise and other fun prizes.

Financial Options
It is our mission to provide you with affordable, high-quality treatment that fits your budget! We offer a variety of payment options to meet your needs. We accept Visa, MasterCard and American Express. We also offer financing options by which monthly payments can be debited directly from your checking account. For customers who pay in full at the beginning of treatment, we offer discounts. We thoroughly explain your options, so that we can accommodate your needs.

Insurance
Our office is committed to helping you maximize your insurance benefits. As a service to our patients, we will bill insurance companies for services and accept payment directly from them. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. We will discuss your insurance benefits with you individually when you come in for a complimentary consultation. If you have any questions, our courteous staff is always available to answer them.

Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment.
